Payroll Deduction Information All regular CUSD employees may purchase unlimited CVIP accounts for $13.50 through an automatic payroll deduction plan. Subscription forms are available at the District Office and at all school offices, as well as online. CUSD employees can also come into the CVIP Office at the corner of Cedar and Barstow and complete a subscription form. Forms will also be mailed or faxed to interested CUSD employees who call 278-1111 and request a CUSD Employee form.
